IUI Treatment :Intra uterine insemination (IUI) involves the extraction of a semen sample from the male partner or a donor, which is directly injected into the uterine cavity around the time of ovulation. IUI is often prescribed as the first in line of infertility treatments when treatment by medication fails. IUI is recommended in the following situations:1. When there is a mild to moderate decrease in the sperm count and motility.2. When no apparent cause for infertility is found (unexplained infertility).Basics to undergo an IUI treatmentApart from a routine examination, the following tests are carried out before performing an IUI1. Semen analysis2. Sonography of the uterus and ovaries3. Tests to check whether the tubes are open [via Hystero-salpingography (HSG) or laparoscopy], the nature of the tests depend on the patient’s age, duration of infertility and clinical findings.Initiation of IUI treatment follows certain planned steps1. First, we stimulate the ovaries with medications. The aim is to produce 2 or 3 eggs in each of the ovaries.2. Then we monitor the response by sonography.3. We trigger the final process of ovulation when the follicle is of a certain size with an injection of HCG.4. The husband is required to give a semen sample on the day of an IUI. Alternatively, a previously preserved sample can also be used. The semen sample is processed to separate the motile and normal sperms.5. Performing the actual IUI – This is a very simple outpatient procedure.– The IUI is then carried out 36 to 40 hours after the trigger injection (HCG) is administered.– The prepared semen sample is injected directly into the uterus using a fine catheter.Precautions to take after an IUI treatmentIUI is a simple procedure and does not require any special precaution. You need to rest for a few minutes after the procedure, after which you can resume all your routine activities. At times, you may also be prescribed medication to support a pregnancy.Intricacies involved in an IUI treatmentIUI is a simple and safe procedure. Apart from mild pain and spotting on the day of an IUI, there are no major complications. Around five to ten percent of the patients have multiple [generally twin] pregnancies. Sometimes, a patient may respond excessively to medications and produce more eggs, putting her at risk for multiple pregnancies and a condition called Ovarian Hyper Stimulation Syndrome (OHSS). If this occurs, your doctor will advise you accordingly and the cycle may be cancelled or converted to an IVF cycle.IUI success ratesAbout 80% pregnancies due to IUI occur within the first three IUI cycles, post which the success rate lowers. We generally recommend a maximum of three IUI cycles, but this depends upon the age of the patient and the cause and duration of infertility.

Medications for Ovarian Stimulation will be prescribed to stimulate the ovaries to produce several eggs in order to increase the chances of pregnancy. There are two protocols namely Long Protocol and Antagonist Protocol available under the IVF treatment at Wings and you will be advised by our doctor as appropriate.– Under Long Protocol, Injections will be started on the 21st day of the previous menstrual cycle. A sonography and blood test is conducted before the actual stimulation is started. You will be asked to come to the Wings center on the second or third day of your next menstrual cycle to start the actual ovarian stimulation.– Under Antagonist protocol, fewer injections and the injections for ovarian stimulation are started from the second day of the menstrual cycle.– In both protocols, injections are to be taken at approximately the same time every day. Though, the injections can be taken at your Home in order to minimize the disruption in your medication, it is advisable to come to the hospital for your injections. Our medical staff will provide instructions on how to administer the injections. It is common to experience some discomfort and mild side effects, while taking these medications.2. The next step is the Ultrasound examination. A vaginal ultrasound examination is conducted five days after the administration of medications to measure the response of your ovaries with regard to the number and size of the follicles. If the response is poor and only one or two eggs are expected, or when the chances of pregnancy are low, we might suggest cancelling the IVF cycle.3. Further, HCG injection is given to trigger the final maturing of the eggs when they are ready for collection. Egg collection is planned approximately 36 hours after the HCG injection. The embryologist will inform you when your husband/partner is required to give his semen sample.4. The next step is Ovum pickup (Egg Collection) where the procedure of removing the eggs from the ovaries is carried out with the help of transvaginal sonography under general anesthesia. The procedure takes about 15 –30 minutes and on an average 8 – 10 eggs are collected from each ovary. You will be able to go Home, three hours after the procedure is completed. It is common to have some vaginal bleeding after this procedure, but that settles down in a day or two. Discomfort as a result of the swelling of the ovaries is common.5. After Ovum is picked, fresh semen sample should be collected on the day of the ovum pick-up (egg collection). However, if a fresh sample cannot be produced on the same day, a previously frozen sample can also be used.– The semen sample is best produced by masturbation – Lubricants should NOT be used as they can affect fertilization – It is sometimes difficult for some men to produce a semen sample on request. In this case, you may collect the sample at Home/elsewhere and bring it to Wings Clinic within an hour of collection. – Unless advised otherwise, abstinence is recommended prior to two days before the egg collection. However, an abstinence of more than four or five days will reduce sperm quality.6. After ovum is picked up and semen is collected, the eggs will be fertilized in the lab either by standard IVF or by ICSI. On the next day, the embryologist will examine the eggs for fertilization.7. Now the egg is fertilized and Embryo is formed. Embryo transfer is performed two or three days after egg collection. The embryo is transferred through the cervix into the uterus via a thin, soft plastic tube guided by sonography. You will be asked to take some injections and/or tablets after the embryo transfer to support the luteal phase. It takes only a few minutes and causes no discomfort.8. You can resume all normal activity including sexual intercourse, after the procedure. However, heavy work such as lifting weights or climbing stairs is not advised.9. Embryo freezing can also be done if there are more than three well-developed embryos, it is possible to freeze them for a later transfer, if necessary. This procedure will be discussed with you before the embryo transfer. Separate consent forms must be signed for embryo freezing.10. A Pregnancy test is conducted approximately two weeks after the embryo transfer where you will undergo a test for beta-HCG to check if gestation has been achieved. If the test is positive, a vaginal ultrasound scan will be performed to confirm this and view the gestational sac. If the beta-HCG test is negative, further treatment will depend on whether you have frozen embryos or not.Precautions to take after an IVF treatment:1. Avoid intercourse– This helps prevent vaginal infections.2. No heavy lifting– Avoid lifting heavy weights. Heavy lifting may strain abdominal muscles that have been stressed by the IVF procedure. Woman should stay away from any tough household work.3. No baths– Many fertility doctors caution women against taking baths during the two-week wait. A bath may introduce a foreign substance into the vagina and potentially cause infection that may lead to miscarriage. Patients are encouraged to take showers instead.4. No strenuous exercise– Women are advised not to do any heavy exercise or aerobics after the IVF procedure. Light exercise such as walking is recommended rather than running.5. Take progesterone– After ovulation, it is necessary to have a proper amount of progesterone to maintain pregnancy. Though a body produces progesterone, doctors will prescribe artificial progesterone in suppository or injection form to make sure that appropriate levels of the hormone are maintained during this time. The injections usually continue until the twelfth week of pregnancy till when a woman’s body begins producing enough of the hormone on her own. ICSI Treatment :Intra-Cytoplasmic Sperm Injection (ICSI) is a process where a single sperm is injected directly into the egg using a fine glass needle. The main objective of ICSI is to ensure that the spermatozoa fertilize the egg membrane. The woman is stimulated with medications and then an egg retrieval procedure is performed so that several eggs can be obtained to attempt an ICSI procedure.ICSI is recommended in cases of severe male infertility, previous fertilization failures with IVF or situations in which there are a limited number of oocytes available.Steps to follow in an ICSI treatmentA very fine delicate needle is carefully inserted through the zona (the shell of the egg) and into the center (cytoplasm) of the egg.The sperm is injected into the cytoplasm and the needle is removed.The eggs are checked the next morning for evidence of normal fertilization.Intricacies involved in an ICSI treatmentThere is evidence that the babies conceived through ICSI may have an increased risk of chromosomal abnormality. The possibility that a boy conceived as a result of ICSI may inherit his father’s infertility. An increased risk of miscarriage because the technique uses sperm that would not otherwise have been able to fertilize an egg.
